Recurring Payment Auth Form
This recurring payment form authorizes the merchant, "Party Services Inc.," to charge future payments periodically to settle any outstanding balance owed by the individual or account holder specified in this form. This payment form will remain active and valid for the life of this account until canceled, and no unpaid balance should remain. This account and the payment agreement will stay in effect until we, "Party Services Inc.," receive our equipment in good condition and without any unpaid invoices. The account holder must complete all required fields below, sign the form, and submit it.

By selecting the “Yes” checkbox, I, as the primary account holder, grant authorization to the business merchant “Party Services Inc.” to allow any individual representing me or affiliated with my business, including partners, colleagues, employees, business managers, friends, and family members, to have the ability to place future orders in my name under this account agreement. They will be permitted to place orders via phone or email, and my account will be held accountable for the payment of these orders.

4. A security deposit will be collected at the time of account opening in the specified amount of --->

5. In order to cancel this account agreement, the account holder must submit a written termination letter to officially discontinue the service. This letter should be sent to us either through email or traditional mail. The account will stay active until all equipment is returned to us or collected by our team. The items must be returned in good, working order; otherwise, we reserve the right to utilize part or all of the security deposit, or even impose additional charges to cover any outstanding balance on the account.
** Please be aware that we are unable to guarantee a fixed price for our helium supply to our customers. Prices may fluctuate at any moment in the near future, which means you could end up paying more than your current rate. The volatility in helium gas pricing and market shifts is beyond our control, as inventory supply and pricing are dictated by the largest gas and supply corporations in the industry.
